An exhibition on biomedical engineering for Vattenhallen Science Center LTH
This thesis contains the basis for an exhibition about biomedical engineering due to be displayed at Vattenhallen in the summer of 2013. The aim of the exhibition is to inspire and interest the children and adults that come and visits Vattenhallen. Hopefully at least one child will feel interested at study at LTH in the future. The basis contains 13 experimental stations and a short description of a potential laboration. Apart from these 13 stations three experiments have been constructed as well. An EMG-controlled prosthetic hand, an ultrasound phantom in the form of a 13 week old fetus and a software program for an experiment about MR/CT imaging. H1N1 vaccination in Sjögren’s syndrome triggers polyclonal B cell activation and promotes autoantibody production
Objectives: Vaccination of patients with rheumatic disease has been reported to result in lower antibody titres than in healthy individuals. However, studies primarily include patients on immunosuppressive therapy. Here, we investigated the immune response of treatment-naïve patients diagnosed with primary Sjögren’s syndrome (pSS) to an H1N1 influenza vaccine. Methods: Patients with Sjögren’s syndrome without immunomodulatory treatment and age-matched and gender-matched healthy controls were immunised with an H1N1 influenza vaccine and monitored for serological and cellular immune responses. Clinical symptoms were monitored with a standardised form. IgG class switch and plasma cell differentiation were induced in vitro in purified naïve B cells of untreated and hydroxychloroquine-treated patients and healthy controls. Gene expression was assessed by NanoString technology. Results: Surprisingly, treatment-naïve patients with Sjögren’s syndrome developed higher H1N1 IgG titres of greater avidity than healthy controls on vaccination. Notably, off-target B cells were also triggered resulting in increased anti-EBV and autoantibody titres. Endosomal toll-like receptor activation of naïve B cells in vitro revealed a greater propensity of patient-derived cells to differentiate into plasmablasts and higher production of class switched IgG. The amplified plasma cell differentiation and class switch could be induced in cells from healthy donors by preincubation with type 1 interferon, but was abolished in hydroxychloroquine-treated patients and after in vitro exposure of naïve B cells to chloroquine. Conclusions: This comprehensive analysis of the immune response in autoimmune patients to exogenous stimulation identifies a mechanistic basis for the B cell hyperactivity in Sjögren’s syndrome, and suggests that caution is warranted when considering vaccination in non-treated autoimmune patients
New graduated nurses satisfaction in their work environment
Introduktion: I Sverige förbereds sjuksköterskorna inför yrkeslivet genom en treårig utbildning som leder till en legitimation. Sjuksköterskans ansvarsområde är omvårdnad samt att främja hälsa. Det råder sjuksköterskebrist på flera platser i världen och framtidsprognosen tyder på att det kommer vara fortsatt brist det kommande decenniet. Syfte: Att belysa nyutexaminerade sjuksköterskors tillfredsställelse i arbetslivet de tre första åren. Metod: En litteraturstudie genomfördes enligt Polit och Becks (2012) flödesschema i nio steg. Litteratursökningen genomfördes i databaserna Cinahl och Pubmed. Artiklarna kvalitetsgranskades utefter Polit och Becks (2012) granskningsmall, 10 artiklar återstod, fem av kvantitativ metod, två av kvalitativ metod samt tre av mixad metod. Resultat: Litteraturstudien gav tre huvudteman som påverkade tillfredsställelsen på arbetet; förväntningar, stöd samt verksamhet. Det visade sig att de nyutexaminerade sjuksköterskorna hade höga förväntningar på sig själva. De upplevde att de inte fick det stöd de var i behov av. Hög arbetsbelastning och låg bemanning var den största bidragande faktorn till låg tillfredsställelse på arbetsplatsen. Slutsats: Det verkar således vara ett stort problem på fler platser i världen att nyutexaminerade sjuksköterskor upplever den första tiden i arbetet som påfrestande. Detta på grund av låg bemanning, kollegor som inte stöttar samt introduktionsprogram som inte är individanpassade. För att uppleva tillfredsställelse och trygghet på den nya arbetsplatsen behöver sjuksköterskorna successivt introduceras till det nya yrket. För att få de nyutexaminerade sjuksköterskorna att vilja stanna inom professionen är det viktigt att skapa trygghet och att de upplever att de får stöttning under den första tiden.
